Pathway to Surgery:

To begin, North Florida Bariatrics requires every patient to attend an informational seminar or view our online informational seminar. Once that is completed you will schedule an appointment with one of our surgeons. It is at that appointment that you and your surgeon will discuss which weight loss surgery is right for you.

Once you have had your consultation with the surgeon you will begin your physician supervised diet required by your insurance company (Note: not all insurance companies require this diet). During that time you will begin to fulfill the medical clearance appointments listed below.

When your physician supervised diet and clearance appointments are complete we will submit all the information to your insurance company to obtain an authorization for surgery. When the authorization is complete you will start your pre-operative diet given to you by our dietitian and you will have a final visit with the surgeon and your surgery will be scheduled.

Medical Clearance for Weight Loss Surgery:

We are committed to helping you be as healthy as you can be for life! It is in your best interest for a medical assessment to be completed prior to surgery so we are well aware of your unique medical needs. We want to know your medical history as well as current state of health. This allows us the opportunity to give you the best possible medical support while you are in our care. These medical assessments help provide us with an understanding of your health:

• Letter of medical necessity from your Primary Care Physician
• Medical clearance from a Physician who is a Pulmonologist
• Medical clearance from a Physician who is a Cardiologist
• Current set of Lab screening tests
• Nutritional Assessment and teaching about by a Registered Dietitian
• Letter of Psychological clearance from a Psychologist or Psychiatrist
• Current update on any past medical conditions
